I am in the process of renewing AST agreement with our existing Tenants. They have been living in the property since last 2 years. The tenants signed 12 months AST contract during each tenancy period with a 6 months break clause.
This time agent advised me that they can’t put 6 months break clause as it would be irrelevant. Agent has advised me that 6 months break clause would only be applicable in the first contract.
Can you please advise if break clause can be added in the subsequent AST renewals with same tenants or not.

Of course it can.
I have no idea why your agents are saying this unless perhaps they want you to have six month tenancy agreements with the tenants, which would allow them to charge more renewal fees.
Or maybe the person at the agent is getting this muddled up with something else.

As Tessa said in the last sentence, I think the agent is getting muddled and I suspect they are under the false impression that the break clause does not get renewed each time in line with the new tenancy.
